Critical Care Inter Hospital Transfer Forms.
It was agreed by stakeholders to introduce standardised critical care transfer form in February 2021. This is the clinical record for all critical care transfer irrespective of the origin of the transfer and requires all fields to be completed.
The form is triple carbonate. The front sheet contains patient information, ambulance information and observation chart; the 2nd sheet provides a risk assessment, pre transfer checklist and contact details for all critical care units in SYB.
Once the transfer is completed the top (White) copy to be placed in patient record in receiving unit, middle (Pink) copy to be returned to WYCCODN office* for audit purposes, bottom (Yellow) copy to be retained by transferring unit and placed in patient records.

Transfer Bags.
All critical care units across the SYB Network have recently received a new transfer bag. Familiarity with environment, processes and equipment reduces the risk inherently associated with critical care transfers. Therefore, the South Yorkshire & Bassetlaw Critical Care Network decided to standardise the transfer bag and contents to improve patient safety.
The bag was designed by the North West London Critical Care Network who have prepared this short video.....
